Description
Responsibilities:
- Manage and coordinate the care of residents utilizing the nursing process.
- Collaborate with team members to formulate individualized resident care plans.
- Assess and document changes in residents' physical or mental status and report findings to the primary care physician and treatment team.
- Provide leadership and direction for direct care partners.
- Establish positive working relationships with team members, residents, and family members to facilitate open, direct communication.
Requirements:
- Nursing degree or diploma from an accredited college, university, or nursing school.
- Long-term care facility experience and specialized knowledge of nursing care of the elderly and rehabilitative nursing preferred.
- Nursing license within the State of New Hampshire.
- Resident Assessment Tool certification.
